Our community health teams (CHTs) are made up of nurses, physiotherapists, occupational therapists and healthcare assistants.
They deliver holistic nursing and therapy services to our patients, along with both planned and unplanned care.
Senior members of the CHTs are responsible for making sure every patient receives the right care to meet their needs.
We work closely with other providers of care - GPs, the acute hospitals, social care, care agencies and care homes. voluntary organisations, hospices, Marie Curie and Macmillan,
There are 15 CHTs working across Suffolk in:
- Mildenhall and Brandon
- Bury St Edmunds Town and Rural
- North East Suffolk
- Ipswich (3)
These teams are led by local area managers who are responsible for providing clinical leadership to the CHTs, hospitals and specialist services.
Community Matrons work alongside our CHTs, providing nursing care to people who have multiple, serious and complex conditions.
The teams also work with our specialist services.
